Request for Prerequisite Evaluation
Loading...
Thank you for your interest in the University of Minnesota PharmD program! As a service to prospective students and applicants, we are happy to review your transcript(s) to determine which of our prerequisites you have completed.
For applicants who have taken coursework outside the United States and Canada, a WES course-by-course foreign transcript evaluation is required (unless the coursework appears with a grade on a U.S. transcript, which is typically how study abroad credit appears). WES is the approved foreign transcript evaluation service used by PharmCAS, our application system. We strongly advise you to order a WES course-by-course evaluation for the purposes of this request for prerequisite evaluation. If you submit this request using another type of foreign transcript evaluation, there is a risk that WES will not evaluate courses in the same manner, which will result in differences in what is considered fulfilled.
Please note that our evaluation of prerequisite status is based on the accuracy and completeness of the documentation you provide to us.
Did you know we have guides of approved prerequisites for over 150 different schools across the United States? Your school might be one of them; visit
z.umn.edu/guide
to see our guides!
Have you checked to see if your college or university is in the guide?

Have you checked to see if your college or university is in the guide?
Yes
No
No worries! Please review our published guide of approved prerequisites at
z.umn.edu/guide
to see if your school has a guide. We find it helpful to look through that first and if there is a guide for your school, you won't have to wait for us, and can do it yourself!
Did you find your school listed in our guides?
Did you find your school listed in our guides?
Yes
No
Thanks for checking! Since we do not have a guide of approved prerequisites for your school, please submit the request below. Our goal is to respond to prerequisite evaluation requests within one week.
Great! Feel free to go through the guide for your school to determine which courses you believe are complete and which courses will fulfill those you still need to take. For most students, this is a straightforward process, but if you have any questions, please contact us at pharmacy@umn.edu. If you find a guide for your school, but prefer admissions staff review your transcripts, you may still upload your transcript by clicking "no" to the question above and uploading your transcript(s).

Please upload your transcripts here - required
Please note that all transcripts must include your name, institution name (no abbreviations), all courses taken, when courses were taken, and grades received. Screenshots from your institution's website or degree evaluations from your student portal will not be accepted. Transcripts that are not up to date, missing pages, cut, and not readable will not be accepted.
